Hospital discharge support

Our hospital discharge service provides housing advice for patients who find themselves at risk of homelessness on discharge from a hospital stay.

This includes working with patients who are behind on rent or needing advice with budgeting and housing maintenance.

The team supports those who have come into hospital directly off the streets, as well as patients who have never experienced homelessness before, and may feel very fearful of what awaits them on discharge.

Helping those facing homelessness with secure accommodation, reduces the likelihood of readmittance into hospital, last year saving the NHS £400,000.
